FSGS/Renal Transplant Program & Apheresis Algorithm

 

Kidney becomes available

 

Renal team contacts BBMD on call

 

BBMD Resident/Fellow on call contacts BBMD Attending Physician

 

Decisions to be made between BBMD and Renal are:

ü      Types of products to use (albumin or FFP)

ü      Type of anticoagulants to use (ACD-A or heparin)

ü      Determine type of access (peripheral, CVC or fistula)

ü      Where procedure will take place (can NOT be in TAU if on off hours, i.e. overnight or weekends)

ü       Determine when the procedure needs to be done

ü      Dialysis should occur prior to TPEX

 

BBMD on call contacts the Apheresis RN on call

 

BBMD on call reconnects with Renal to finalize plan where they:

ü      Determine who the MD/Team in charge of patient,

 It should be Surgery because patient is pre-op

 

Renal Team communicates plan to patient &Transplant Surgical Team

 

BBMD writes the orders and consents the patient for Apheresis (if consent not already obtained)

Premedicate with Benadryl/Tylenol strongly recommended if replacing with FFP

 

Apheresis RN or BBMD order the product through Blood Bank

 

Apheresis RN contacts patient’s Nurse for Report

Equipment and products prepared for treatment

Procedure started and completed
